Perioperative Patient Blood Management (excluding obstetrics): Guidelines from the French National Authority for Health

Anaesth Crit Care Pain MedPublished Clinical practice guideline

Recommendations

As issued by the authors

  1. Preoperative: detect anemia and iron deficiency and correct well in advance of surgery, when possible, in surgery with moderate to high bleeding risk or known preoperative anemia

  2. Intraoperative: TXA and some surgical techniques to limit bleeding in high bleeding risk or hemorrhage

  3. Intraoperative: cell salvage in some surgeries with a major risk of transfusion

  4. Postoperative: limit blood samples, but monitor postoperative anemia, which may lead to corrective measures (IV iron in particular) or more precise diagnostic assessment

  5. Postoperative: "restrictive" transfusion threshold considering comorbidities and, most importantly, patient tolerance

  6. Implement a patient blood management (PBM) strategy and program throughout the perioperative period in healthcare establishments to reduce blood transfusion and LOS
